1 002例汞作业工人职业健康检查状况分析

Occupational health examination of 1 002 workers occupationally exposed to mercury

  • 摘要: 目的 了解汞作业工人职业健康检查情况。 方法 选取汞接触组1 002例,对照组805例,主要进行血压、心电图、谷丙转氨酶、尿常规、血常规、肾功能(尿素氮和肌酐)、尿汞和尿β2-微球蛋白等检查,并进行统计学比较。 结果 作业场所共检测60个点,汞的时间加权容许浓度均<最高容许浓度(0.02 mg/m3)。汞接触组和对照组高血压以及心电图、谷丙转氨酶、血常规、尿常规、肾功能、尿汞各项指标异常检出率比较,差异均无统计学意义(P>0.05);而尿β2-微球蛋白异常与对照组比较,差异有统计学意义(P<0.01)。尿β2-微球蛋白水平、尿汞浓度和工龄均呈正相关(rs=0.995 8、0.952 2,P<0.05)。 结论 汞接触对职业工人有累积作用。汞对作业工人的肾功能有早期影响,主要表现为尿β2-微球蛋白的增高,故汞接触工人在岗期间职业健康体检时做尿β2-微球蛋白项目很有必要。

     

    Abstract: Objective To investigate adverse effects of occupational exposure to mercury on workers. Methods Totally 1 002 workers occupationally exposed to mercury were studied and their health examination data were compared with 805 workers without any records of exposure to known occupational hazards. they were received examination of blood pressure, electrocardiogram,alanine aminotransferase (ALT),routine blood test, routine urine test, kidney functions(BUN and B-Cr),urine mercury and urine β2-MG. Results The mercury concentrations at 60 measured workplaces were lower than 0.02 mg/m3. The blood pressure, electrocardiogram, urine routine tests, blood routine test, kidney functions, and urinary mercury showed no significant changes compared with those in the non-exposed workers(P>0.05). However, urine β2-MG of exposed workers was higher (P<0.05). urine β2-MG and urinary mercury concentrations of exposed workers were both related to employment time (r=0.995 8,0.952 2, respectively, P<0.05). Conclusion Urine β2-MG is an earlier indicator for change of kidney functions caused by mercury. It is necessary to examine urine β2-MG when we do occupational health examination for mercury exposed workers.
