尘肺病患者大容量肺灌洗术后肺功能改善情况的螺旋CT评价

Evaluation of pulmonary function improvement in pneumoconiosis patients after whole-lung lavage using spiral CT examination

  • 摘要:
    目的 应用螺旋CT评价尘肺病患者大容量肺灌洗(whole lung lavage, WLL)术后肺功能改善情况的效果。
    方法 选取经WLL术治疗的尘肺病患者共50例, 治疗前和治疗后3 d均行肺功能和螺旋CT检查, 记录检查结果并进行对照。
    结果 两组患者肺功能检查指标中, 仅中晚期组患者第1秒用力呼气量(FEV1.0)、残气量(RV)、肺总量(TLC)、RV/TLC等指标在WLL术前后差异有统计学意义(P < 0.05)。而两组患者所有螺旋CT指标在WLL术前后差异均有统计学意义(P < 0.05)。
    结论 螺旋CT较肺功能检查更能准确反映尘肺病患者WLL术后肺功能的改善情况, 可作为肺功能评估的印证和补充。

     

    Abstract:
    Objective To evaluate the improvement of pulmonary function in pneumoconiosis patients after whole-lung lavage(WLL)using spiral CT examination.
    Methods Pulmonary function test and spiral CT examination of 50 pneumoconiosis patients were performed before and 3 days after WLL treatment.
    Results The pulmonary function test showed that only the patients at midterm and later stage had improvement of FEV1.0, RV, TLC and RV/TLC after the WLL (P < 0.05), but the spiral CT examination showed that all patients had improvement of pulmonary function after WLL(P < 0.05).
    Conclusion Spiral CT examination is more sensitive than lung function examination to show the improvement of pulmonary function in patients with pneumoconiosis after WLL and could be used as an complementarity in evaluation of pulmonary function.
