二氧化氯吸入致急性中毒6例救治体会

Treatment experience of six cases with acute poisoning caused by inhalation of chlorine dioxide

  • 摘要: 二氧化氯(ClO2)常用于漂白剂和饮用水消毒剂,可分解成次氯酸、氯气、氧气。国内曾有二氧化氯溶于水后中毒事件的报道,但吸入气态二氧化氯致急性中毒的患者少见。本报道中患者均为吸入气态二氧化氯后中毒,部分患者出现急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS),需体外膜肺氧合治疗,所有患者经治疗后痊愈。气态二氧化氯中毒主要累及呼吸系统,及时脱离中毒环境、抗炎症反应及呼吸支持、抗感染是治疗的关键;重症者大多于24 h内出现呼吸衰竭需机械通气,对常规机械通气无效、心功能良好的危重患者,可采用静脉-静脉体外膜肺氧合(veno-venous extracorporeal membrane oxygenation,V-V ECMO)替代肺脏功能。
