船舶制造业装配工人腕/手部工作相关肌肉骨骼疾患影响因素分析

Analysis of influencing factors for work-related musculoskeletal disorders of the wrist/hand among assembly workers in shipbuilding industry

    目的 探讨船舶制造业装配工人腕/手部工作相关肌肉骨骼疾患(WMSDs)的影响因素,为预防和控制腕/手部WMSDs提供依据。
    方法 2023年3—12月,采用便利抽样法选择辽宁省和广东省386名船舶制造业装配工人作为研究对象,采用《中文版肌肉骨骼疾患调查问卷》调查其腕/手部WMSDs的发生情况,采用二元logistic回归模型分析腕/手部WMSDs可能的影响因素。
    结果 回收有效问卷382份,有效回收率98.9%。船舶制造业装配工人腕/手部WMSDs发生率为12.6%。多因素logistic回归分析结果显示:相对于从不/有时搬运重物(每次> 20 kg)的装配工人,经常/频繁搬运重物(每次> 20 kg)者发生腕/手部WMSDs的风险升高(OR = 22.354);相对于从不/有时使用振动工具的装配工人,经常/频繁使用振动工具者发生腕/手部WMSDs的风险升高(OR = 42.060);相对于从不/有时以不舒服姿势工作的装配工人,经常/频繁以不舒服姿势工作者发生腕/手部WMSDs的风险升高(OR = 8.201);相对于从不/有时加班的装配工人,经常/频繁加班者发生腕/手部WMSDs的风险升高(OR = 8.407);相对于从不/有时需要用手捏紧物品或工具的装配工人,经常/频繁需要用手捏紧物品或工具者发生腕/手部WMSDs的风险升高(OR = 28.219);以上差异均有统计学意义(P<0.05)。
    结论 不良作业姿势和不合理工作组织制度是船舶制造业装配工人发生腕/手部WMSDs的主要影响因素,改善不良工效学姿势、提供适合的工具或设备、合理安排加班有助于降低船舶制造业装配工人腕/手部WMSDs的发生风险。

     

    Abstract:
    Objective To investigate the influencing factors for work-related musculoskeletal disorders (WMSDs) of the wrist/hand among assembly workers in the shipbuilding industry and to provide a basis for the prevention and control of such disorders.
    Methods From March to December 2023, a total of 386 assembly workers from the shipbuilding industries in Liaoning and Guangdong provinces were selected using a convenience sampling method. The occurrence of wrist/hand WMSDs was assessed using the Chinese version of the Musculoskeletal Disorders Questionnaire. A binary logistic regression model was employed to analyze potential influencing factors.
    Results A total of 382 valid questionnaires were collected, with a valid response rate of 98.9%. The prevalence of wrist/hand WMSDs among these workers was 12.6%. Multivariate logistic regression analysis showed that assembly workers who regularly/frequently handled heavy loads (> 20 kg per lift) had an increased risk of wrist/hand WMSDs (OR = 22.354), compared with the workers who never/seldom handled heavy loads; the workers regularly/frequently using vibratory tools had an increased risk of wrist/hand WMSDs (OR = 42.060), compared to the workers never/seldom using such tools; workers regularly/frequently working in uncomfortable postures had an increased risk of wrist/hand WMSDs (OR = 8.201), compared to the workers never/seldom working in uncomfortable postures; the workers working regularly/frequent overtime had an increased risk of wrist/hand WMSDs (OR = 8.407), compared to workers working never/seldom overtime; compared with assembly workers never/seldom gripping objects or tools by hand, the workers regularly/frequently gripping objects or tools by hand had an increased risk of wrist/hand WMSDs (OR = 28.219). All the above differences were statistically significant (P < 0.05).
    Conclusions Poor working postures and unreasonable work organization were the main factors contributing to wrist/hand WMSDs among these shipbuilding assembly workers. Improving poor ergonomic postures, providing appropriate tools or equipment, and arranging overtime more reasonably may help reduce the risk of wrist/hand WMSDs among this occupational group.
