一例疑似过量照射放射工作人员外周血染色体畸变分析

Analysis of chromosome aberrations of peripheral blood in a radiation worker with suspected overexposure

  • 摘要: 报告1例因外周血淋巴细胞染色体畸变异常而疑似为职业性辐射损伤,但最终确诊为先天性嵌合体核型的案例。受检者初检显示染色体断片增多且畸变率高达43%,结合个人剂量监测值超标,易误判为过量的职业照射所致。经深入溯源发现,该剂量异常系个人剂量计违规佩戴导致的数据失真,非真实受照;进一步吉姆萨显带核型分析的结果显示受检者体内存在两种细胞核型:45,X27与46,X,+mar33,后者细胞数目虽正常,但出现短小的片段类似无着丝粒断片,其断片实为遗传性标记染色体(mar)构成的嵌合体。本案例提示,在放射工作人员职业健康检查中,面对染色体畸变异常,应注意与先天性染色体异常进行鉴别,必要时可通过吉姆萨显带染色体核型分析确定畸变来源,以有效防止误诊。
