Analysis of influencing factors of work-related musculoskeletal disorders among workers in metal manufacturing enterprises in Yangpu District, Shanghai

    目的 调查上海市杨浦区金属制造行业工人工作相关肌肉骨骼疾患(WMSDs)的发生情况并分析其影响因素。

    方法 采用流行病学横断面调查方法,选择中国疾病预防控制中心统一编制的《肌肉骨骼疾患调查表(中文版)》调查2024年杨浦区金属制造业劳动者近一年WMSDs的发生情况。运用logistic回归模型对WMSDs可能的影响因素进行多因素分析。

    结果 共收集966份调查问卷,回收有效问卷843份,有效回收率为87.27%。调查对象WMSDs的发生率为53.02%(447/843)。多因素logistic回归分析结果显示:与20~29岁的劳动者相比,年龄为40~49岁(OR=1.702,95% CI:1.080~2.683)、≥50岁(OR=2.404,95% CI:1.435~4.027)的劳动者发生WMSDs的风险增加;工作时间>8 h的劳动者WMSDs的发生风险是工作时间<8 h的3.215倍(95% CI:1.486~6.955);工作环境涉及寒冷凉风或气温变化(OR=1.935,95% CI:1.376~2.721)、休息后继续工作(OR=2.772,95% CI:1.647~4.666)、岗位人员短缺(OR=1.871,95% CI:1.354~2.587)等因素均可能增加劳动者WMSDs的发生风险(P<0.05)。与身高<170 cm的劳动者相比,身高170~180 cm(OR=0.554,95% CI:0.386~0.796)和身高>180 cm(OR=0.416,95% CI:0.244~0.712)者WMSDs的发生风险更低;此外,和同事轮流完成工作(OR=0.490,95% CI:0.359~0.668)、每班休息次数>2次(OR=0.634,95% CI:0.449~0.895)、休息时间充足(OR=0.566,95% CI:0.413~0.774)等因素也可能降低WMSDs的发生风险(均P<0.05)。

    结论 上海市杨浦区金属制造业工人WMSDs发生率高,其发病风险受个体因素、工作相关因素等诸多因素影响。建议相关企业优化工作环境及轮班制度,加强职业健康教育工作,履行职业病危害因素告知义务。劳动者要提高自身健康意识,以降低WMSDs的发生风险。

     

    Abstract:

    Objective To investigate the prevalence of work-related musculoskeletal disorders (WMSDs) among workers in the metal manufacturing enterprises in Yangpu District, Shanghai, and to analyze associated risk factors.

    Methods A crosssectional study was conducted. The data was collected using the standardized Chinese Musculoskeletal Disorders Questionnaire developed by the Chinese Center for Disease Control and Prevention to survey WMSDs occurring among metal manufacturing employees in 2024. Multivariable logistic regression was applied to analyze multiple factors associated with WMSDs.

    Results A total of 966 questionnaires were collected, of which 843 were valid, with a valid response rate of 87.27%. The prevalence of WMSDs among participants was 53.02% (447/843). Multivariate logistic regression analysis revealed that, compared with workers aged 20 to 29 years, those aged 40 to 49 years (OR = 1.702, 95%CI: 1.080 to 2.683) and ≥ 50 years (OR = 2.404, 95%CI: 1.435 to 4.027) had significantly higher risks of WMSDs. The daily working time > 8 hours increased the risk 3.215-fold compared with daily working time < 8 hours (95%CI: 1.486 to 6.955). Other factors that increased WMSD risk included exposure to cold drafts or temperature fluctuations in the work environment (OR = 1.935, 95%CI: 1.376 to 2.721), resuming work immediately after breaks (OR = 2.772, 95%CI: 1.647 to 4.666), and staff shortages (OR = 1.871, 95%CI: 1.354 to 2.587) (all P<0.05). Conversely, workers with a height of 170 to 180 cm (OR = 0.554, 95%CI: 0.386 to 0.796) and ≥ 180 cm (OR = 0.416, 95%CI: 0.244 to 0.712) had lower risks than those < 170 cm. Additionally, the factors included alternating tasks with colleagues (OR = 0.490, 95%CI: 0.359 to 0.668), taking more than 2 breaks per shift (OR = 0.634, 95%CI: 0.449 to 0.895), and having sufficient rest time (OR = 0.566, 95%CI: 0.413 to 0.774) were associated with reduced risk (all P<0.05).

    Conclusions The prevalence of WMSDs among workers in the metal manufacturing industry in Yangpu District, Shanghai, was relatively high, which was associated with multiple factors, including individual characteristics and work-related factors. Recommendations for enterprises included optimizing and improving the work environment, refining shift scheduling systems, strengthening occupational health education, and fulfilling the duty to inform workers about occupational hazard factors. Workers should enhance their health awareness to reduce the risk of developing WMSDs.
