2015—2019年昆山市院前急救跨区域服务及急救反应时间分析

Analysis of cross-regional prehospital emergency service and response time in Kunshan City from 2015 to 2019

      目的  探索昆山市院前急救跨区域服务存在的问题,为提升该城市院前急救服务水平提供依据。
      方法  基于昆山市院前急救中心2015—2019年154 234条急救个案数据,使用R软件4.0.4版本,利用桑基图对转运流程做可视化展示;比较各区镇患者同区域和跨区域的急救转运情况,分析昆山市急救反应时间延长的可能原因。
      结果  2015—2019年,昆山市承担急救任务较多的站点主要有急救中心、开发区分站和张浦急救站;急救患者较多的区镇主要有玉山镇、开发区和周市镇。同区域转运患者96 597例(占62.63%),跨区域转运患者57 637例(占37.37%)。不同区镇跨区域转运患者的构成比不同(P < 0.01),未建立本区域内的急救站点的淀山湖镇和周庄镇跨区域转运患者占比最高。昆山市同区域转运平均急救反应时间为12.40 min,跨区域转运平均急救反应时间15.65 min。各区镇跨区域转运患者的平均急救反应时间均长于同区域的反应时间(P < 0.01)。各区镇在急救过程中反应时间达标(≤12 min)的转运患者共有87 286例,占56.80%。急救反应时间≤12 min的转运患者中,同区域转运患者65 059例(占74.54%),急救反应时间 > 12 min的转运患者中,同区域转运患者31 207例(占47.01%)。
      结论  昆山市院前急救反应时间与是否跨区域转运密切相关。需要合理布局急救站点,在无急救站点的地区设置相应的急救站点,缩短急救反应时间,提高急救效率。

     

    Abstract:
      Objective  To understand the problems of cross-regional prehospital emergency services in Kunshan City, and to provide basis for improving the prehospital emergency services.
      Methods  The data of 154 234 first aid cases in Kunshan prehospital emergency center from 2015 to 2019 were analyzed, using R software version 4.0.4 and Sankey diagram to visually display the transfer process. The intra-regional and inter-regional first-aid transportation of patients in different districts and towns were compared, and the possible reasons for delay of first-aid response time in Kunshan City were analyzed.
      Results  From 2015 to 2019, the stations that undertook major first aid tasks in Kunshan City mainly include the First Aid Center, the Sub-station in the Development Zone and Zhangpu First Aid Station. Most emergency patients were mainly from Yushan Town, the Development Zone and Zhoushi Town. Totally 96 597 patients (62.63%) were transported within the same region, and 57 637 patients (37.37%) were transported across regions. The proportion of inter-regional transferred patients in different districts and towns was different (P < 0.01), and the highest were in Dianshanhu Town and Zhouzhuang Town where there have no emergency stations established. The average time of intra-regional first-aid response of Kunshan City was 12.40 minutes, and the average time of inter-regional first-aid response was 15.65 minutes. The difference was of significance (P < 0.01). The first-aid time of 87 286 patients (56.80%) was less than 12 minutes (specified time limit) and among these 65 059 patients (74.54%) were intra-regionally transported. Among the patients with emergency response time > 12 minutes, 31 207 patients (47.01%) were intra-regionally transported.
      Conclusions  The reaction time of prehospital first aid in Kunshan City was closely related to intra-regional or inter-regional first-aid transportation. First aid stations need to be reasonably arranged, and corresponding first aid stations should be set up in areas without first aid stations, so as to shorten the first aid response time and improve the aid efficiency.
